Before Your First Visit

  1. Check on our web if we take your insurance.
  2. List your concerns to discuss.
  3. Write your past and present medical issues.
  4. List of your medications.
  5. Fill out the patient forms before your first visit.
  6. Bring your insurance card and a photo ID.
  7. Bring a medical referral if needed.
  8. Don’t forget any x-rays, MRIs, or lab tests relevant to your issues.
  9. Arrive 15 minutes early on your first visit.
