Welcome to CT Physical Therapy Care! Making that first appointment is the most important step toward living a pain-free life. We know you might be in pain or anxious about what comes next, so we have made our intake process as simple as possible.
Our goal is to spend less time on paperwork and more time focusing on your recovery. Please review the checklist below to ensure your first session is smooth and effective.
Your Pre-Visit Checklist
To ensure you get the full benefit of your dedicated treatment time, please prepare the following before you arrive:
1. Complete Your Patient Forms
We value your time. Instead of filling out paperwork in the waiting room, we recommend completing your intake forms online or printing them at home. This allows our team to review your medical history before you even walk through the door.

2. Check Your Insurance & Benefits
Understanding your coverage brings peace of mind. While we accept most major insurance plans (including Workers’ Compensation and No-Fault cases), we highly recommend confirming your specific benefits, copays, or deductibles.

3. Gather Your Medical Documents
To give you the best care, we need the full picture of your health. Please bring:
- Your Photo ID and Insurance Card.
- A doctor’s referral or prescription (if applicable).
- Any relevant X-rays, MRI reports, or lab test results.
- A list of current medications.
4. Dress for Movement
Physical therapy is an active process! Please wear loose, comfortable clothing (like gym wear) and sneakers. We need to be able to easily access the area of your body where you are experiencing pain.
5. Arrive Early
Please arrive 15 minutes early for your first appointment. This gives our front desk team time to process your registration so your therapist can see you promptly at your scheduled time.
